A copy of the letter of admission/proof of enrollment (such as an early acceptance letter) from a postsecondary institution.

Please attach a letter written by someone you have volunteered for, or who knows you well and can speak about the good volunteer work you do (not a relative).

A copy of the diagnosis from a medical/psychological professional.
