|About the Survey:|
In 2017, Autism Ontario launched our Caregivers' Survey. We took what you told us and communicated it to service providers, government representatives, professionals and community groups. This year we are launching our first annual Provincially Speaking Survey! Based on your feedback, we have put together new targeted questions to gather information from both caregivers and adults with ASD about the status of autism services in Ontario in 2018.
Your participation in this survey is completely voluntary and your responses will be protected as private and confidential.
The results of this survey will be shared with relevant stakeholders for informational and academic purposes. Autism Ontario wants your voice to be heard and we will share results with you on our website as well. Your responses will be combined with those of all survey participants: therefore, unless you specifically agree, none of your personal responses will be shared, providing you with complete anonymity.
This survey will take approximately 30 minutes to complete. You may also save your answers as you go along, and return to the survey at any time to complete it. The survey will be open until June 5th, 2018.
At the end of the survey as a thank you, you will have the option of being entered into a draw to win one of three Amazon 100$ gift cards!
