The provincial government has posted an IEP template for parents & students to access at:

This is an excellent way for parents to see what an IEP looks like as well as what information and wording is used. There are samples including one for children with autism.
The GECDSB is currently using a similar template. They plan to move to the provincial template within 1-2 years.
