How to Submit Receipts:

Once you have paid for your child's program, you may submit your receipts to Autism Ontario Wellington to receive your reimbursement.

  • To submit your receipt by email, send it to wellington@autismontario.com
  • To meet in person, please contact us by email or message us on facebook to arrange a time to meet to submit your receipts. 
  • If you would like to submit your receipts by mail you may send them to our Provincial Office:  AUTISM ONTARIO (Wellington Chapter. Attn: Brittany Sherwood.) 1179 King St W. Suite 004. Toronto, ON M6K 3C5

Please Note:

  • Proof of Diagnosis must be provided before any receipts will be processed. 
  • All receipts must be submitted by September 31, 2018. 
  • Reimbursement cheque will be mailed to the address you provided on your application within 30 business days of submitting receipts.


1:1 Support Funding

If you are using your funding for 1:1 Support and require a receipt template please click here

Please Note: 
Support Worker receipts should include the name and contact info of your support worker, as well as total amount paid signed by both parties.
