Chapter Waiver & Photo Waiver
Participation in Autism Ontario Upper Canada Chapter hosted events, activities and Social Learning Opportunities requires signing of a Chapter Activity Waiver. Please print out the following Chapter Activity Waiver, sign and bring it with you to the event. A separate waiver is required for each Autism Ontario hosted event.
To save time, we suggest you save the blank waiver to your computer, input your family information and save. When you attend an event hosted by Autism Ontario Upper Canada Chapter, just print out the completed Waiver, fill in the date and activity, and sign in the appropriate places.
We request that you complete a Photo Waiver for each participant, however, this is not mandatory to participate in Autism Ontario Upper Canada Chapter hosted events, activities and Social Learning Opportunities. Only one Photo Waiver needs to be completed. It will cover all activities unless revoked.
