CAMH- Dual Diagnosis Program
Our current project looks the types of health services used by adolescents and adults with
ASD and the related experiences of family members (namely parents) who provide care
to these individuals.
- Participation is completely voluntary and parents can choose to withdraw at any point.
- You will be asked to fill out a consent form as well as some background information. Link to online consent form: http://www.surveymonkey.com/s/Family_Study_Consent
- We will then contact you every two months for one year to better understand the types and frequency of healthcare services you are using, as well as your satisfaction with those services.
- Surveys can be completed online, by mail or over the phone.
